Confusing ADHD in Adult Women with Other Conditions

For many women, the path to ADHD runs through other diagnoses first. Anxiety, depression and sometimes personality disorders are commonly identified before ADHD is considered. 

This happens because the diagnostic criteria focus on the visible, hyperactive symptoms common in boys and men. ADHD symptoms in women more often surface internally, as difficulty regulating emotions, overwhelm or chronic worry. Severe emotional dysregulation in particular is often mistaken for bipolar disorder or borderline personality, though for many women, the underlying cause is ADHD.

The Australian ADHD Professionals Association's clinical practice guidelines now formally acknowledge that ADHD may be under-recognised in women due to the issues described above. But for most women, an accurate diagnosis still comes a decade or more too late.

Masking in Women with ADHD

Women with undiagnosed ADHD often develop a long-term habit of compensating for executive function difficulties through perfectionism, people-pleasing and over-preparation. This is commonly known as “masking,” and it can unconsciously cover up ADHD.

Over the years, masking can become intertwined with a woman's personality and sense of identity. Masking is exhausting to maintain, which may contribute to the related mental health challenges often experienced by women with ADHD.

ADHD Symptoms in Women

The most common signs and symptoms of ADHD in women aren't the visible hyperactivity of the childhood stereotype. They're quieter, internal and often carefully concealed.

Common experiences include:

  • A constant sense of mental noise that doesn't switch off (often described as multiple thoughts cycling at once, like "fifty browser tabs open" in the mind)

  • Persistent difficulty starting tasks, particularly administrative or unstimulating ones (even when the consequences matter)

  • Forgotten appointments and missed deadlines, despite genuinely caring and trying

  • Working at full effort just to perform at the level others appear to manage easily

  • Strong emotional responses that feel out of proportion, especially around perceived criticism or rejection (sometimes called rejection sensitive dysphoria)

  • Periods of intense focus on something interesting, followed by complete depletion

  • Difficulty winding down to sleep, even when the body is exhausted

What many women describe most clearly is the gap between how they appear and how things feel internally. With high-functioning ADHD in women, you might appear competent and organised on the outside, but this requires constant effort and causes near-constant overwhelm within.

Inattentive ADHD in Women

Women with ADHD are more likely than men to have a predominantly inattentive-type presentation. 

Inattentive ADHD typically involves daydreaming, difficulty sustaining attention, lost trains of thought mid-sentence and the ongoing experience of starting things without finishing them.

Where hyperactive features do show up in women, they often manifest internally - as racing thoughts, restless energy, or difficulty regulating emotions - rather than visible physical hyperactivity.

ADHD and Female Hormones

ADHD is fundamentally a condition involving dopamine regulation. For women with ADHD, it’s important to know that estrogen, one of the main female sex hormones, also happens to be a key regulator of dopamine in the brain.

How Estrogen Impacts Dopamine

At a brain chemistry level, estrogen affects dopamine in three main ways:

Boosts Production & Availability - Estrogen increases the activity of tyrosine hydroxylase, the enzyme that synthesises dopamine.

Reduces Breakdown/Reuptake - Estrogen reduces the rate at which dopamine is broken down and reabsorbed, allowing it to remain active in the brain for longer.

Enhances Receptor Sensitivity - Estrogen increases the density and sensitivity of dopamine receptors (particularly D1 and D2 subtypes), which support cognitive function.

Your appointment is conducted via secure video call, with no software to install. 

 

During the session, your doctor will:

Common Questions About ADHD in Women

  • Until recently, ADHD was diagnosed almost exclusively in childhood, using criteria built around how the condition presents in boys. Girls with ADHD were systematically missed. Many of those girls are now adult women, often coming to assessment as adults due to increased awareness of the signs of ADHD in women.

  • The underlying condition is the same, but specific ADHD traits in women often differ. Women are more likely than men to present with predominantly inattentive symptoms rather than the externalised hyperactivity associated with the childhood stereotype.

  • There are three recognised presentations: inattentive, hyperactive-impulsive, and combined. Women are more likely than men to have the predominantly inattentive presentation, which often involves daydreaming, mental restlessness and difficulty completing tasks, rather than visible hyperactivity.

  • Current evidence does not support adult-onset ADHD as a distinct condition. ADHD is a neurodevelopmental condition that has been present from childhood, even in cases where it is identified for the first time later in life. What changes in adulthood is often the level of structural support available. The cognitive demands of work, family and life-stage transitions can also surface difficulties that were previously compensated for.

  • Estrogen plays a substantial role in supporting dopamine function, and ADHD is fundamentally a condition of dopamine regulation. Recent Australian research has confirmed that the large majority of women with ADHD experience meaningful symptom variation across the menstrual cycle, postpartum and through perimenopause and menopause. Familiarity with these patterns is part of how our doctors approach assessment and ongoing care.

  • The predominantly inattentive presentation is the most common subtype seen in adult women. Many women with ADHD have never been visibly hyperactive at any age. Even where hyperactive symptoms are present, they often show up internally as racing thoughts or restless energy, rather than as visible physical movement.

  • ADHD presents in girls similarly to how it presents in adult women - quieter, more internal, often missed by school-age referral pathways. Our ADHD assessment covers adolescents from age 15 and adults of all ages.
